2001 ford f150 rough idle when cold outside

Help!. 04 F-150 XLT 4x4 with the 4.6. w/rough idle. It had 5,700 miles on it at purchase, and now has 19,000. Soon after purchase, the rough idle was noticed. Noticable in park, worse in drive when waiting at a light. Gets better with A/C or defroster on. Multiple times to the dealer, one TSB done with no success. MAF cleaned, and new air filter.

When the engine is started cold, regardless of the outside temperature, the idle is rough and there is a noticable power loss. However, once the engine reaches FULL operating temperature the idle is smooth and power returns. During the rough idle, I notice the tach needle quivering and that the engine is revving lower than normal.
when engine is dead cold ( first run of the day ) she runs just fine until she reaches ~ 1/2 on the temp gauge. i changed the plugs but will pull and she is a bit more tolerant and runs better on warmer days . some times turning off and restart lets it run in the nigher RPMs ok , but will still rough idle if warm .
